Here is a sample of the form I created for my 20time students to help keep track of their blogs. The most important aspect is that students post the COPY of their blog here too, so I can skim the submissions. Go to the form itself.
If you're interested, I geeked out with the submission spreadsheet to include some conditional formatting that identifies if any of the fields need attention or do not meet the requirements. I also created a formula that counts the words in the copy field and warns me if a submission is under the word count. Click on the image to make a copy of the spreadsheet for yourself.
